California Partnership for Access to Treatment

The California Partnership for Access to Treatment (CPAT) is a diverse network of advocacy organizations, community groups, health care providers and employers, committed to ensuring a healthy and productive California.

CPAT serves as a communications, education and resource network to keep its partners and the communities they serve informed about critical issues involving access to treatment in California.

Medicare Part D Open Enrollment Toolkit

The Medicare prescription drug coverage (Part D) annual open enrollment period, from October 15th to December 7th, allows eligible beneficiaries to ensure they have the best plan in place to meet their drug needs at the lowest cost. This is your chance to review your current health plan and make sure all your plans, including your Medicare Part D plan, meet your health needs for the upcoming year.

Many seniors and individuals with a qualifying disability rely on Medicare Part D to affordably and easily access their medications. Whether you're selecting a plan for the first time or looking to update your current plan, the Medicare Today fact sheet offers tips and resources to assist you with choosing a drug plan, including:

  • Making a list of your medication for the year
  • Checking your coverage
  • Making sure your preferred pharmacy is in your network
  • Knowing your drug costs
  • Reviewing plan ratings

For background information and frequently asked questions about the enrollment period, check out the Medicare Access for Patients (MAP) Rx open enrollment brochure. There, you can also compare plans and learn how to determine your eligibility for Medicare Part D Plans and Extra Help.

To learn more about the prescription drug plans that are available in your area, check out the Medicare Plan Finder at www.medicare.gov/find-a-plan. You can also call Medicare at 1-800-MEDICARE (1-800-633-4227) or visit www.medicare.gov to enroll.

Affordable Care Act Health Insurance Exchange Toolkit

CPAT is pleased to provide our partners and advocates with an Affordable Care Act Health Insurance Exchange Toolkit to assist you in educating your network to choose the best plan and coverage for prescription medicines in essential health benefits.

The Affordable Care Act ensures health insurance and prescription medication coverage is available to all Americans, beginning in 2014. The coverage can be maintained through a government program such as Medicare or Medicaid, an employer, an insurer, or through a policy purchased on a state health insurance exchange.

The resource is intended for you to use to educate your networks on the health plans offered through the exchange, including:

  • Overview of plan designs;
  • Financial assistance in the exchange marketplace;
  • Key components of plan selection; and
  • Coverage for prescription medicines in essential health benefits.

We encourage you to make this toolkit your own by branding it with your organization logo or putting it on letterhead. Our team is happy to assist any of our partners or advocates in branding this toolkit and disseminating to your network.
